How to apply for UDID card

• Persons with disabilities who already have a disability certificate (whose data has been migrated to the UDID portal): All PwDs who have been issued a disability certificate go to Swavlambancard.Gov.In/Pwd/Searchappationation. To search your details by beneficiary ID/state ID or Aadhaar number (if linked). On receiving your details, fill and submit the application.
• A person with a disability already has a disability certificate (whose data has not been migrated to the UDID portal): visit Swavlambancard.Gov.In. Click Apply Online UDID, fill the application a new, and select the “Disability Certificate ?: Yes,” option in the Disability Details tab. Fill in the other details and submit the application.
• The disabled person does not have a disability certificate: they can go to Swavlambancard.Gov.In and then fill the online application by clicking on the Apply For Disability Certificate & Unique disability ID link.
